The March 2019 Quality Improvement Workshops are now open for registration!

At this dual track interactive workshop, you will learn population health strategies and services which promote the health and wellness of your patients. These services improve the health of your community and increase practice revenue while improving quality scores and ACO performance.

Make Population Health Practical

  • Develop Sustainable Programs
  • Improve Quality by Reducing Variation
  • Target Patient-specific Interventions

Work on your challenges to Advanced Initiatives

  • Collaborate with peers and Caravan Health specialists

Excel at Promoting Interoperability

  • Understand the changes that are new this year
  • Develop plans to address program requirements

Prepare for 2019 Quality Work

  • Understand how to maintain and improve quality scores
  • Improve patient care

You will learn about updated guidelines and quality measures for the 2019 year and have the opportunity to network and learn from fellow Caravan Navigators regarding best practices. You will leave this workshop with a new foundation or deepened understanding of how to implement financially sustainable population health programs to support your patients and physicians most effectively.

Advanced Session: This portion of the Spring Quality Improvement Workshops is only for facilities that have achieved an adequate threshold of Annual Wellness Visit performance. You will talk with Caravan experts and your fellow peers about best practices and overcoming barriers and struggles. When you register for this session you will be questioned about your current Plan, Do, Study, Act process to help the Caravan team lead the discussion during our Advanced Session. Your facility must have a 20% Annual Wellness Visit rate to qualify for this session.

Registration and Travel Reimbursement Guidelines


ACO Clients and AIM/ACO Clients 
must register at least 1 attendee to participate.

ACO clients are not eligible at this time for travel reimbursement to the Quality Improvement Workshops. If you have any questions or need clarification, please contact EducationalEvents@CaravanHealth.com


PTN/TCPI Clients may register up to 3 total attendees per clinic to participate.

The participant is eligible for reasonable reimbursement for travel expenses associated with TCPI-required travel to quarterly Quality Improvement Workshops.

Reimbursement is for up to three (3) attendees total regardless of number of clinics. Reasonable travel reimbursement will include meals according to the U.S. General Services Administration (GSA) per diem and mileage. If your facility is over 250 miles (one-way) from your Quality Improvement Workshop, airfare and hotel accommodation for one night may be optionally submitted for reimbursement.

All reimbursement must be submitted using NRACC's Standard Reimbursement Template (download HEREwithin 30 days of costs incurred. For your reference, please find the GSA Per Diem Guide on our Pre-Launch Resources page or download HERE. To submit for reimbursement following your travel, you will fill out and send our Standard Template Invoice to AccountsPayable@CaravanHealth.com. If you have any further questions, please contact EducationalEvents@CaravanHealth.com.

CPC+ Clients may register up to 3 total attendees per clinic to participate.

CPC+ clients are not eligible at this time for travel reimbursement to the Quality Improvement Workshops. If you have any questions or need clarification, please contact EducationalEvents@CaravanHealth.com.




Our classes offer a combination of lecture and practical exercises, determined by the individual instructor, on focused aspects of population health strategies. Your fellow participants will have a range of healthcare knowledge and background, from beginners to people with years of industry experience. Caravan Health & National Rural Accountable Care Consortium instructors, participants, and administrators all work together to create a welcoming, supportive environment.

Workshop Cancellations

Caravan Health reserves the right to cancel a workshop/class when the required minimum enrollment is not met. Earliest notification will be given and the participant will receive a full workshop ticket refund when applicable. If a facilitator needs to cancel a workshop/class or there is an inclement weather delay or closing, attendees registered in the workshop/class will receive an email notification at the email address with which they registered. We will give as much notice as we can about cancellations.
