Description: This session showcases how to apply health literacy insights to a statewide diabetes prevention and management initiative. The UMD Horowitz Center for Health Literacy collaborates with multiple state and local public agencies and private sector groups to define and implement diabetes prevention activities that integrate health literacy tools and techniques. Speakers will address policy, clinical services, community-based coalitions, and social support to help community members take steps to prevent or manage diabetes. Objectives: - State the elements of a socio-ecological approach to a health literacy and diabetes initiative. - List at least 2 activities that show how to apply health literacy tools and techniques to diabetes prevention or management. - Describe the role community coalitions can play in health literacy and diabetes initiatives.
