Nearly all clinicians ask about chronic disease co-morbidities during a patient history; however, it requires additional clinical ability to provide integrated care for co-morbid conditions when patients receive physical therapy services. Regardless of experience level, this session will help attendees take the next step when providing wholistic care to enhance patient health. Attendees will learn the physical therapy opportunities that exist in current evidence-based guidelines for diabetes mellitus, hypertension, high cholesterol, and weight management. In this highly interactive session, attendees will learn methods to administer guideline-supported interventions when patients present with these chronic conditions as co-morbid health concerns. Attendees will enhance their ability to manage multiple co-morbid conditions using numerous real-world patient cases. This session will be especially impactful because attendees will help each other navigate the common barriers of time, patient expectations, work system factors, or payment issues that frequently frustrate clinicians when they provide integrated, evidence-based physical therapy for the whole patient. Whether you’re a student or an experienced clinician, this session will be relevant to your practice.
