Bridging Community Support and Clinical Care Within a 638 Behavioral Health System: The CWOY Model

This presentation will highlight how Teca Kici Okijupi - Connecting With Our Youth (TKO-CWOY) uses the flexibility of Oyate Health Center’s 638 alongside clinical care for Indigenous youth and families in the He Sapa service area. The session will focus on how Support Navigators strengthen suicide prevention, intervention, postvention, and continuity of care through outreach, one-on-one support, care coordination,
transportation support, follow-up, community engagement, and connection to resources. Presenters will discuss how this relationship based model complements OHC Behavioral Health services by helping reduce barriers between clinical care, families, schools, hospitals, and community partners while supporting youth identified as at risk for
suicide.
