Transforming Tribal Oral Health: Care Circles and the Care Progression Pathway

This presentation describes how the Oyate Health Center Dental
Department redesigned its approach to care using Care Circles and a
structured Care Progression Pathway. Instead of treating isolated dental
problems, care is organized in a purposeful sequence that prioritizes
relationships, coordination, and long-term outcomes. By defining the
order of care and engaging the entire care team—providers, assistants,
families, and community—the model focuses on eliminating oral disease
and guiding Relatives toward sustained oral health.
