Mistakes and errors are bound to occur, and some will escalate to include financial and legal consequences. This session investigates the etiologies of negative events and assesses the effects on the practice’s culture.

Procedural errors and clinical mistakes are frequently reported in veterinary medicine. Such can have detrimental effects on patient safety, trust between caregivers, and client confidence. The severity of the event may also cause professional, financial, and legal outcomes. Frequently, negative events can be reduced to common risks that predisposed the error; and sometimes it is just a “perfect storm of events” which by themselves were harmless. This session examines those etiologies and how they affect the practice’s culture.
