The Nepean Blue Mountain region’s ageing population is set to grow significantly, with projections indicated that 22% of the population will be aged over 65 by 2041. Older people without adequate social connections are at an increased risk of experiencing poorer mental health and wellbeing, negatively impacting on older people’s physical health and use of health services. Primary care is ideally placed to identify those at risk of loneliness, and prescribe social activities to support these people.
In 2019, Wentworth Healthcare (WHL) piloted the ‘Improving Social Connections for Older People’ which involved a multifaceted model and an asset-based community development strategy. This comprised community co-design, governance, mapping, establishment of Health Connectors in general practice and enhanced community capacity to appropriately respond to lonely and socially isolated older people. The Connector Program since expanded to include Connector Points, Wellbeing Connectors and Intergenerational Programs. Connectors can use WHL’s My Health Connector directory as a tool to support accessing local social, health and lifestyle activities in our region.
WHL conducted a Connector Evaluation from 2024 – 2025 with ARDT. Initial findings resulted in 96 older adults receiving a social prescription to connect them with activities in their local community. Program data showed that on average, quality of life, measured by the WHO-5 Well-Being Index, increased from intake to follow-up assessment. For participants who completed both intake and follow-up surveys, paired comparisons showed that the vast majority reported an improvement in quality of life.
Recently, WHL expanded the Connector Program to include commissioning local pharmacies as Pharmacy Connector Points and a Pharmacy Connector. Local pharmacy staff complete the Pharmacy Addressing Loneliness and Social-isolation (PALS) training to recognise and assist vulnerable people experiencing loneliness and socially prescribe activities, services or programs that may help them to build their connections and support networks.
