Oyate Health Center is strengthening how we care for Relatives by building Integrated Care Teams that bring medical, dental, behavioral health, and support services together in a more connected way. This work is grounded in our commitment to whole-person care, strong relationships, and meeting Relatives where they are. By focusing on communication, shared responsibility, and early response to needs, we’re working to create a care experience that feels coordinated, respectful, and easier to navigate.
This session will share how we’ve used the Southcentral Foundation (SCF) Integrated Care Team playbook to guide our approach—translating core principles into day-to-day practice within a tribal health setting. We’ll walk through how teams are structured, how roles are defined, and how tools like daily huddles and care coordination processes are used to support both staff and Relatives. We’ll also share lessons learned along the way, including where things have been challenging and what’s helped move the work forward.
Participants will leave with clear team frameworks, adaptable templates, and implementation guidance built on relationships and core connections. This session is meant to be real and useful—grounded in experience, rooted in connection, and aligned with a shared vision of care that honors tribal values, builds trust, and supports better outcomes.
