Dr Ebony Lewis is a Geriatrics Emergency Nurse and an emerging international leader in frailty measurement, assessment and prevention for older people in hospitals, general practice, and Indigenous populations. With Dr Cardona and a team of social and health professionals and consumers engaged in informal caregiving, she has co-designed a self-efficacy and social prescribing intervention for older socially frail carers which is the topic of their presentation today.
