Reimagining Health: Supporting Complete Care
from Systems to Surroundings 


EACH25 Conference, a trans-disciplinary conference, brings together leaders and practitioners in innovative social prescribing interventions.  These are based on environment, daily activity, and meaningful connections with a common goal to improve health and wellness. They include art, heritage, culture, sport, leisure, nature, education and access to basic human essentials which contribute to better health outcomes and stronger communities.

The conference seeks to build an epistemic community that bridges disciplinary, geographic, and cultural boundaries. Together, we aim to exchange insights, forge strategic partnerships, and lead the way toward a future healthcare model where prevention, social wellbeing and community empowerment and are at the forefront of health policy and practice.


Conference highlights include

  • Keynote Speakers and Panel Discussions
    Listen to international and national experts in social prescribing implementation; social prescribing research and impact assessment; integrated care; health and social care policy making; and place-based service innovation.
  • Networking Opportunities
    Engage with peers, leaders, and innovators across different sectors.


Call for Abstracts EACH25

We invite contributions from people and organisations reimagining what health care looks like across sectors. Whether you’re in general practice, education, community services, arts, housing, youth work, planning or policy, if your work supports people to live well, this conference is for you.


Themes may include:

              • Creative and cultural health interventions
              • Connection and belonging as protective factors
              • Youth-focused and intergenerational approaches
              • Embedding prevention into mainstream care pathways
              • Using local assets and lived expertise to shape health
              • Equity, inclusion, and the right to thrive across life stages


              We especially welcome submissions focused on:

                        • Children, young people, and families
                        • Arts, culture, identity, and creative health
                        • Meeting basic needs through care (housing, food, transport, income)
                        • Culturally grounded, place-based and community-led models
                        • Innovation in prevention, systems design, funding, and commissioning
