Reimagining Health: Supporting Complete Care
from Systems to Surroundings 


EACH25 Conference, a trans-disciplinary conference, brings together leaders and practitioners in innovative social prescribing interventions.  These are based on environment, daily activity, and meaningful connections with a common goal to improve health and wellness. They include art, heritage, culture, sport, leisure, nature, education and access to basic human essentials which contribute to better health outcomes and stronger communities.

The conference seeks to build an epistemic community that bridges disciplinary, geographic, and cultural boundaries. Together, we aim to exchange insights, forge strategic partnerships, and lead the way toward a future healthcare model where prevention, social wellbeing and community empowerment and are at the forefront of health policy and practice.


Conference highlights include

  • Keynote Speakers and Panel Discussions
    Listen to international and national experts in social prescribing implementation; social prescribing research and impact assessment; integrated care; health and social care policy making; and place-based service innovation.
  • Networking Opportunities
    Engage with peers, leaders, and innovators across different sectors.


Call for Abstracts EACH25

We invite contributions from people and organisations reimagining what health care looks like across sectors. Whether you’re in general practice, education, community services, arts, housing, youth work, planning or policy, if your work supports people to live well, this conference is for you.


Themes may include:

              • Creative and cultural health interventions
              • Connection and belonging as protective factors
              • Youth-focused and intergenerational approaches
              • Embedding prevention into mainstream care pathways
              • Using local assets and lived expertise to shape health
              • Equity, inclusion, and the right to thrive across life stages


              We especially welcome submissions focused on:

                        • Children, young people, and families
                        • Arts, culture, identity, and creative health
                        • Meeting basic needs through care (housing, food, transport, income)
                        • Culturally grounded, place-based and community-led models
                        • Innovation in prevention, systems design, funding, and commissioning

                        A/Prof Christina Aggar

                        Dr. Christina Aggar is Associate Professor Nursing at Southern Cross University and Conjoint Research Academic at Northern NSW Local Health District. She conducts interdisciplinary research to improve health service delivery and workforce development.

                        Prof Thomas Astell-Burt

                        Thomas Astell-Burt is the Professor of Cities and Planetary Health at the University of Sydney’s School of Architecture, Design and Planning. He is also an Australian Research Council Future Fellow.

                        A/Prof J.R. Baker

                        Dr JR Baker is an experienced executive with 20 years’ experience strategically leading health organisations to achieve cutting-edge service delivery.

                        Prof Genevieve Dingle

                        Genevieve Dingle is a Professor in Clinical Psychology. Her research areas include mental health, music, loneliness, and social connection. She has developed programs to enhance mental health and well-being for diverse populations.

                        Prof Xiaoqi Feng

                        Professor of Urban Health and Environment in UNSW’s Faculty of Medicine and Health and an Honorary Professorial Fellow at the George Institute for Global Health, Australia. Prof Feng is the co-creator of the Lonelygenic Environments Conceptual Model

                        Dr Rosanne Freak-Poli

                        Dr. Rosanne Freak-Poli is a life-course epidemiologist. Her work is strongly driven by social justice, being the distribution of wealth, opportunities, and privileges within a society.

                        Jenny Kirschner

                        Jenny Kirschner brings over two decades of experience in healthcare & pharmacy. Jenny is the founder of PALS (Pharmacy Addressing Loneliness and Social-isolation) and is in the leadership team for the International Social Prescribing Pharmacy Association

                        Robin Mellon

                        Robin is CEO of Better Sydney, a Director at the UN Global Compact Network Australia, and Project Manager in the Modern Slavery Working Group at the Property Council of Australia

                        Assistant Prof Kate Mulligan

                        Dr. Kate Mulligan is an Assistant Professor in Social and Behavioural Health Sciences in the Dalla Lana School of Public Health at the University of Toronto. She is a globally recognized expert in community approaches to health and wellbeing.

                        Dr Kuljit Singh

                        National Chair RACGP, Social Prescribing Special Interests Group. Clinical Director Projects, KnG Healthcare currently running a Social Prescribing project TLC@SPLASH. General Practitioner & Past Asst Professor General Practice, Bond University.

                        Leanne Wells

                        Leanne is a Director at the Australian Commission on Safety and Quality in Health Care, Specialist Adviser and former Chief Executive Officer of the Consumers Health Forum of Australia, the nation's preeminent peak body for health care consumers.

                        Prof Yvonne Zurynski

                        Yvonne leads the Health System Sustainability Stream at the Centre for Health Resilience and Implementation Science (CHRIS) at the Australian Institute of Health Innovation, Macquarie University.
